New Client Pet Records

Welcome new clients! Your first visit to All Creatures Veterinary Clinic in Wasilla, Alaska establishes a vital foundation for our relationship with you and your pet. We will gather important background information, like your pet’s records and medical history, and give you time to get to know the veterinarian.

New Client Forms

Please fill out these forms so we can expedite your first visit. You can send the completed form(s) to acvcfrontdesk@gmail.com or bring it to the clinic at your appointment time.

Please fill out our New Client Form.
If you are taking your pet out of state, fill out our Health Certificate Form.
If you are leaving your pet with a caretaker, fill out a Vacation Permission Form so your pet can receive medical care if needed in your absence.
For curbside appointments, fill out our Curbside History Form.

What to Expect

Being prepared for your first appointment will ensure that the doctor has all of the needed information to provide the best possible care for your pet.
Take some time to review our staff pages and familiarize yourself with our doctors, technicians and assistants, and receptionists.
All pets should be on a leash (make sure that their collar is not too loose) or in a carrier. If your pet is aggressive, please bring or ask for a muzzle. For the safety of our staff, please let us know before we examine or handle them.
